SESSIONS:
Each session is 45-60 minutes. Please arrive 10 minutes prior to your appointment on your first visit to complete paperwork. Extended appointments are charged in 15 minute increments of the usual fee. These are subject to prior arrangements.
CASH FEES:
60 Minute Session $ 130.00
45 Minute Session: $ 110.00
Any written reports: $50 per page.
Acceptable Payments: Cash or Credit Card (Visa or Mastercard )
INSURANCE:
I am currently accepting Blue Shield, Magellan, Anthem Blue Cross, Anthem EAP, MHN, Kaiser, Beacon Health, Military One Source, IEHP and Tri-Care. Please call ahead of time with your insurance information readily available. Benefits will need to be verified prior to your first visit. Co-payment due once services are rendered.
Appointments cancelled with less than a 24-hour notice or missed appointments are charged at cash-pay rate.
